Registration Information
Welcome to UTM Accessibility Services! Please ensure you read the information below before starting the registration form.
This online registration form must be fully completed in order to progress to the next steps in the registration process. For questions that follow, those with an asterisk (*) are mandatory. If you forget to answer these specific questions, you will be prompted to complete missed questions before your form can be submitted successfully. You are also able to upload your medical documentation/reports (including psychoeducational assessments and Individual Education Plans) at the end of the form. Important information about medical documentation requirements can be found at https://www.utm.utoronto.ca/accessibility/future-uoft-students/documentation-requirements. If you are unable to upload your documentation/reports, please drop them off at the Accessibility Services main office in Room 2240, Davis Building within the Student Services Hub.
Once you have submitted your form, you will be contacted by Accessibility Services staff within 2 business days (excludes weekends and holidays). Please ensure you upload or drop off your medical documentation/reports at least 2 days before your appointment.
