Registration Information

Welcome to UTM Accessibility Services! Please ensure you read the information below before starting the registration form.

This online registration form must be fully completed in order to progress to the next steps in the registration process. You are also able to upload your medical documentation/reports (including psychoeducational assessments and Individual Education Plans) at the end of the form. Important information about medical documentation requirements can be found at https://www.utm.utoronto.ca/accessibility/future-uoft-students/documentation-requirements. If you are unable to upload your documentation/reports, please drop them off at the Accessibility Services main office in Room 2240, Davis Building.

Once you have submitted your form, you will be contacted by Accessibility Services staff within 2 business days (excludes weekends and holidays) and an appointment will be set up with an Accessibility Advisor. Please ensure you upload or drop off your medical documentation/reports at least 2 days before your appointment.
